How to Access Physiotherapy Treatment with Aviva

1. Contact Aviva

If you would like to make an appointment with us using your Aviva Health Insurance please give Aviva a call first. You can let them know you wish to attend our clinic using our provider number.

Aviva will provide you with a claim number which you can use to book your appointments with us. Once you have this simply give us a call on 01744907012 and we will get you booked in.

2. Book Your Appointment

You can book your session by calling us at 01744907012 or by emailing info@ontimephysio.co.uk. When booking, please provide your Aviva membership number, authorisation or claim number, excess amount, and session allowance.
If you're seeking treatment for multiple conditions, you're welcome to provide more than one authorisation code. Our team will manage your bookings and billing accordingly.
